I think issue 2 was quite illuminating in the nature of the multiple Reinhardts.
While we have seen two different John Reinhardts, the Mirror Event seemed to indicate that there are more, be they mental or real. The combination of Multiple Reinhardts with the Elder Gods and the flashback of tentacles seems to indicate outside influence on the good Doktor.
My theory, let's call it the Vessel Sleepless Hypothesis, is that John Reinhardt was mentally invaded as a child by Elder Gods (or their minions). As he grew up, the ideas from beyond began to invade his consciousness, oozing their way out in the way of inventions and ideas.
The Grinder subculture is based on extreme body modifications. Most Lovecraftian invasions involve the modifications of the worshiper bodies (like the followers of Dagon). Is it possible that even at his beginnings John Reinhardt was trying to get people ready for the incoming event? If not by the physical body modification than surely by the idea.
The meme of grinding sets up peoples minds to accept the idea of modification. It paves the way for the mutations and deformities to come under the rule of the Elder Gods.
Remember, Heavenside is where the faithful went. Perhaps Doktor Sleepless has returned to Heavenside to give them a new faith and prepare the way for the new gods.
When John visited Don Bastardos he was looking for ayahuasca. The reports of use of ayahuasca are varied, but many of the trips involve visits to a particular city (R'yleh or similar?) with similar details of landmarks and geography. If the Elder Gods are working their way into Reinhardt's head, then visiting them on their own grounds may help clear things out. Or more likely, open their way further into his head, causing the split from John Reinhardt into Doktor Sleepless.
When Doktor Sleepless spoke with the mirror, it mentioned not only multiple Reinhardts, but the necessity of choosing this body/persona as the only possible option. This vessel is the one fit for habitation by the Elder Gods, allowing them to further their agenda.
When you look at it, Doktor Sleepless is very much a Prometheus figure. The question is, who did he steal the fire from?

I think I was a bit wrong on the last entry. I don't think Doktor Sleepless is prepping Heavenside for invasion. I think he's using the weapons of the enemy.
Somehow he survived the tentacle assault. Maybe that was his warning. Maybe he saw the future wasn't moving fast enough to combat the Elder Gods. That's why he went on sabbatical. His "head [wasn't] big enough". He needed more access to figure out their plans, and plan a counter-attack, or at least a good defense.
One of the warning signs is Incoming Eschaton Event. Eschaton is usually used as a religious based end of the world. Maybe the signs aren't just for fun.
